What the index measures

Each provider in the directory has an editorial record listing every plan it publishes, with the price we captured, the date and the page we read it on. For each lane the index takes one number per provider: its cheapest plan that includes the medication itself, normalised to one month and including any mandatory membership fee. Prepaid multi-month plans count at their per-month rate. Dose-dependent pricing uses the starting dose.

The cheapest figure is the lowest of those numbers; the median is the middle one, so a single outlier cannot move it. The high figure is the most expensive provider's cheapest plan, which bounds the spread. A provider appears in a lane only if it is live in the directory and publishes a comparable price there.

What is left out, on purpose

Introductory first-month discounts never count as the price. Membership-only fees where the medication is billed separately (Ro, Noom's branded track) are excluded rather than shown at a number that is not the drug's. Brand-name Wegovy and Zepbound plans are not indexed because most providers simply pass through the manufacturer's self-pay price, which is shown separately below. Prices we could not verify on the provider's own site are marked as unverified on the profile and stay out of the index.

Records are re-verified on a published schedule and the index is frozen once a month, so a provider changing its price mid-month moves the live figure but not the archived one. How we collect prices describes the whole process.

The index is free to cite and republish with attribution. Link to this page rather than to a screenshot so readers reach the current month and the method, and quote the month with the number, since prices move.

Price index: questions

How often is the GLP-1 Price Index updated?

Provider records are re-verified continuously and the index is frozen once a month. The live figures on this page reflect the newest capture dates; the archive table keeps each month's frozen numbers so a later price change does not rewrite history.

Why is the median higher than the prices I see advertised?

Advertised prices are usually introductory first-month offers or the price before a membership fee. The index uses the regular monthly rate of each provider's cheapest plan with the medication, membership and shipping included, so it reflects what a second month actually costs.

Is compounded GLP-1 medication cheaper than brand-name?

At the starting dose the gap is narrower than most people expect: LillyDirect sells Zepbound at $299 for 2.5 mg and NovoCare sells Wegovy pens from $199 for the first two fills, while the median compounded tirzepatide plan costs over $200 a month. At maintenance doses brand-name prices rise and compounded prices mostly do not, which is where the saving appears. See Zepbound without insurance and Wegovy without insurance.

Can I use these numbers in an article or a report?
