What an online weight-loss program is buying you

Almost every provider on this page sells the same three things in different proportions: a prescription, the medication, and the support around it. The prescription comes from a licensed clinician who reviews an online intake; the medication is either an FDA-approved brand drug or a pharmacy-compounded version; the support ranges from a messaging thread to a full behaviour-change programme with coaching and an app.

That is why the monthly prices below vary by a factor of ten. A plan that bundles compounded semaglutide, the clinician and shipping into one fee looks nothing like a membership that buys the clinical service while you pay the manufacturer separately for the drug. Our tables normalise both to what leaves your account each month.

The programmes worth paying more for are the ones doing work you would otherwise do yourself — chasing a prior authorisation with your insurer, managing dose escalation, handling side effects. The ones worth less are charging a membership for a prescription you could get elsewhere at the same drug price.

Before you pick one

Check what happens at your maintenance dose, not your first month. Introductory offers are excluded from our prices for exactly this reason: a $79 first month becomes $179 or $299 by the time you are titrated up, and that is the number you will live with.

Check the cancellation terms before the sign-up page, not after. The single most common complaint against every provider in this category — on Trustpilot, on the BBB and in the FTC's 2026 case against one of them — is not the medicine. It is people who could not stop being billed.

And check whether the price includes the drug at all. What each provider costs without insurance sets the manufacturers' own self-pay prices against what these programmes charge, which is the fastest way to see what the membership is really for.

The question most programmes answer badly

What happens when you stop. GLP-1s work while you take them, and weight regain after stopping is common enough that the maintenance plan matters as much as the starting price — yet it is the thing least likely to appear on a pricing page. A programme built around coaching and behaviour change has an answer to that; one that is a prescription pipeline does not.

It is worth asking before you sign up: what does the plan look like at month twelve, what happens to the price if you move to a maintenance dose, and what support continues if you come off the medication.
