That is the same price for the single-dose vial and the multi-dose KwikPen. Direct, or through a telehealth provider?
For Zepbound specifically, buying direct is usually the cheaper route, because Lilly sells to self-pay patients at the same price a telehealth platform pays and the platform adds its own fee on top. What a provider is selling you is the prescription, the clinical review and the follow-up — not a discount on the drug.
That makes the useful question a different one: what is the membership worth to you? If you need a prescriber, want dose management and side-effect support, or would rather one company handled the whole thing, the $74 to $149 a month that telehealth platforms charge buys that. If you already have a prescriber willing to write it, LillyDirect's price is the floor.
Watch the outliers in the table below. Several providers list Zepbound at four figures a month — at or near the $1,087 list price — which is what you pay if nobody is applying Lilly's self-pay programme on your behalf. There is no reason to pay that.
What changes the price
Dose. Zepbound is priced per dose, not per prescription, so the monthly cost rises as your clinician titrates you up: $299 at 2.5 mg becomes $449 at 10 mg and above. Budget for the maintenance dose, not the starting one.
The 45-day refill window. At 7.5 mg and higher, Lilly's $449 price applies only if you refill within 45 days of the previous fill. Miss it and the same box is $499 or $699 depending on dose — a difference of up to $250 for being late.
Compounded tirzepatide is the other lane people consider, and it is genuinely cheaper — but it is a pharmacy-made product that is not FDA-approved, legal only as a patient-specific preparation now the shortage has ended.
