Two lanes, two different products

Brand-name tirzepatide is Zepbound or Mounjaro: FDA-approved, made by Lilly, sold self-pay from $299 a month. Compounded tirzepatide is made by a state-licensed pharmacy to a prescription, is not FDA-approved, and is sold by telehealth providers for considerably less — the table below shows what each charges all-in.

The price gap is real, and so is the reason for it. Compounded drugs do not go through the FDA's review of safety, effectiveness or manufacturing quality, and since the FDA declared the tirzepatide shortage resolved in late 2024, compounding it is lawful only as a patient-specific preparation rather than a copy of the brand. Reading the prices honestly

Compare like with like. A compounded plan that includes the medication, the clinician and shipping in one monthly figure is not comparable to Lilly's drug-only price, because Lilly's price assumes you already have a prescriber. Add a telehealth membership to the brand-name route and the gap narrows.

Then watch the dose. Lilly's brand prices rise with dose and most compounded plans do too, so the number that matters is what you will pay at your maintenance dose, not your first month. Introductory offers are excluded from our tables for that reason.

Do the arithmetic before deciding the gap is as big as it looks. Zepbound at $299 assumes you already have a prescriber; add a telehealth membership of $74 to $149 a month for one, and the brand-name route lands between roughly $373 and $448 all-in. Several compounded plans still undercut that by more than $200 a month — but the comparison is $373 against $169, not $299 against $169.

What you are accepting at the lower price

Compounded tirzepatide has not been through the FDA's review of safety, effectiveness or manufacturing quality, and the agency has warned publicly about dosing errors and adverse events involving compounded GLP-1s, some requiring hospitalisation. That is not an argument that every compounded product is unsafe; it is the reason the price is different.

It also matters who made it. The better providers name their compounding pharmacy and will supply a certificate of analysis on request; the weaker ones describe the product only under their own brand name, which the FDA has specifically called out as obscuring the source.
